Required Health Exams
Illinois State Board of Education School Code requires the following examinations:
Physical Examination & Immunizations
Required before Early Childhood, Kindergarten, 6th and 9th grades and all students enrolling in a new school district. The physical exam must be completed within one year prior to the first day of school and should be filed with the school nurse by July 30th. Required immunizations include diphtheria, pertussis, tetanus, poliomyelitis, measles, rubella, mumps, meningitis, hepatitis B, and varicella.
Sports Physicals
Required for students in grades 6, 7, and 8 who try out or participate in interscholastic sports activities including cheerleading. The child health exam required for grade 6 substitutes as the sports physical. The sports physical is good for one year, plus one month, from the date of the exam.

Dental Examination
Required for students in Kindergarten, 2nd, and 6th grade. The dental exam must be returned to school by May 15th but may be completed within 18 months prior to the May 15th deadline.
Vision Examination
Required for children in kindergarten and all students enrolling in a new school district. The vision exam must be returned to school by October 15th and must have been completed within 12 months prior to the October 15th deadline.
